About the Role

Overview

PathWays has been serving individuals with developmental disabilities and their families in Sullivan and Lower Grafton

counties of NH for over 50 years. Our Self-Directed Service Coordinators work in partnership with individuals, and those

who support them, in designing, implementing, and monitoring supports and services that promote individual choice and

decision-making authority. This position requires a dynamic individual with the ability to work collaboratively within

an individuals team, has excellent time management skills, and a passion for supporting individuals to pursue their best
