Job Summary:

The Providence Center Assertive Community Treatment (ACT) Team is looking for a motivated, team-oriented Registered

Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical

complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and

addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity.
