POSITION SUMMARY

The Physician provides high-quality primary medical care, including evaluation, diagnosis, treatment, and management of

acute and chronic health conditions for patients across the lifespan. This role participates in the development and

implementation of clinical programs, supports care coordination, and contributes to an integrated, team-based model of
