About the Role

JOB DESCRIPTION

This position is responsible for providing personal care and homemaking services to home care and hospice patients under

the supervision of an RN. It documents the various aspects of care on appropriate forms, communicates pertinent patient

information and changes in schedule to RN/supervisor, provides and assists personal cares, range of motion, exercises,

and other simple procedures according to the plan of care, performs incidental housekeeping according to plan of care,

provides companionship/respite (Home Support and Hospice only), provides assistance with medication (Home Support only),
