About the Role

CERTIFIED NURSE ASSISTANT

Trinity Health PACE ministers exclusively to the elderly through a coordinated care model which provides preventive,

primary, acute and long-term care services designed to assist older individuals to live as independently as possible

while continuing to live within their community. This position will be housed primarily at Trinity Health PACE

Alexandria, but will require home care visits and a weekend rotation.
