About the Role

CHILDREN'S LONG TERM SUPPORT (CLTS) PROGRAM SPECIALIST

The Children's Long Term Support (CLTS) Program Specialist is responsible for completing intake and calculating

eligibility for children who are referred to the CLTS program, conduct outreach and assessments for eligible/potentially

eligible children, and enter children into the Eligibility and Enrollment Streamlining System. This position is

responsible for collaborating with multiple systems, internal partners, and community resources. Children's Long Term

Support Case Managers or Support and Service Coordinators (SSC) Support and promote the Mission, Vision and Guiding
