About the Role
CONFLICT FREE CASE MANAGEMENT MODEL CASE MANAGER
You will work collaboratively with the Executive Office of Health and Human Services (EOHHS), Department of Human
Services (DHS), and/or The Department of Behavioral Health, Developmental Disabilities & Hospitals (BHDDH) to provide
case management services to participants who are part of the Conflict Free Case Management Model. The Case Manager
provides in-home visits, monthly outreach, person-centered plan development, oversight, referrals, and monitoring. The
case manager assists with care coordination to ensure participant's needs and wants are addressed using a personcentered
