About the Role

You will be developing clinically based treatment relationships with clients (persons diagnosed with serious and
persistent mental illness or SPMI), to assist them with integrating into the community using a structured recovery plan:
psychiatric, medical, housing, substance abuse treatment, legal coordination and other services.
On a typical day, you will be driving to various locations to meet with your clients and stakeholders in the community
