Care Coordinator

About the Role

Lutheran Social Services of WI and Upper MI is currently seeking a Forensic Care Coordinator for our Operating Avenues

for Re-Entry Success (OARS) program serving Marathon, Wood and Clark counties. This is a full-time, benefit eligible

role. The position requires regular visits in the client's home and/or community and then is able to work from home for

documentation and meeting purposes. The role is primarily, Monday through Friday, 1st shift, but will require

flexibility to meet the needs of clients.

This role provides comprehensive person-centered case management. This requires ongoing learning in Motivational
