St. Cloud VA Health Care System is seeking a Hospice and Palliative Care Advanced Practice Nurse to care for Veterans in
St. Cloud, MN.
The incumbent practices in Rehabilitation and Extended Care (REC) Integrated Clinical Community (ICC) Service and is
responsible for the assessment, planning, coordinated provision, and evaluation of individual treatment plans for
Veterans receiving care from the Hospice Palliative Care program.
